Read the structure
第- forms ordinal numbers, while 老- and 小- can attach to surnames, kinship-like ranks, or established words. In address, 老 and 小 may signal familiarity, seniority, or relative age rather than literal oldness or small size. These are bound word-forming uses, so social context and lexical convention restrict each combination. Keep the model 第 + numeral; 老/小 + surname, rank, or lexical base visible while you compare each sentence.
她住在第二层。

Choose the right context
Form ordinals with 第 directly before a numeral. Use established 老 + surname or rank forms when the relationship and register license them. Use established 小- words or familiar address forms where relative youth, affection, or lexical convention is clear.
老王今天不在办公室。
Old Wang is not in the office today.
Respect the boundary
Do not use 两 after 第; ordinals use 第 + 二. Do not add 老 or 小 to an unfamiliar person's surname without considering politeness and local convention. Do not interpret every 老- or 小- compound literally as old or physically small.
她住在第两层。

Repair the pattern
两 follows the ordinal prefix: Ordinals use 第 + numeral, and the numeral two is 二 rather than 两 in this pattern. A classifier splits 第 from its numeral: 第 attaches directly to the ordinal quantity 三次. An extra 个 cannot split or duplicate that quantity phrase.
她住在第二层。
